Why Koh Samui Is Different from Bangkok for Lab Results

This is something we are upfront about with patients. Koh Samui does not have an on-island molecular lab running same-hour PCR the way Bangkok hospitals do. PCR samples travel to a certified mainland lab. That adds time but does not reduce accuracy.

What it means in practice: 24 to 48 hours is the realistic minimum for PCR results here. For most tourists, that is workable. If you need same-day PCR, you would need to be in Bangkok. For everything else, the island logistics are manageable if you test early enough in your stay.

Rapid tests are not affected by any of this. HIV, syphilis, and hepatitis results are ready in the clinic room while you wait.

Testing Too Early: How the Window Period Affects Your STD Results

This is the part most people miss. If you test too soon after an exposure, even the best test can give you a false negative. That does not mean the test failed. It means your body has not yet produced enough antibodies for the test to detect.

For HIV, the 4th generation rapid test picks up most infections from around 18 to 45 days after exposure. Testing the morning after a risky encounter will almost certainly come back negative, regardless of what actually happened. The result is not reassuring. It is just too early.

For chlamydia and gonorrhea, PCR can detect infection as early as two to five days after exposure, which is a much shorter window. Syphilis antibodies can take up to six weeks to show reliably.

A positive rapid test is a preliminary result. It needs to be confirmed with PCR before anything is definitive. We say this clearly because the anxiety after a positive rapid result is real, and patients deserve a straight answer about what it means.

In the meantime, we discuss your options, including starting PEP if the exposure was recent. PEP must begin within 72 hours to be effective, so do not wait for confirmatory results if that window is closing.
