HPV in Koh Samui — The Most Common STI You Have Probably Never Been Tested For

HPV, human papillomavirus, is the most common sexually transmitted infection in the world. Most people who have it never know. There is no standard STI panel that routinely checks for it. For women on Koh Samui, a Pap smear with HPV DNA testing is the clearest way to find high-risk strains early. For men, Gardasil 9 vaccination is the single most effective step you can take.

You get your STI check after a holiday fling, the results come back clear, and you move on. But HPV was almost certainly not on that panel. This is not a gap in your care. It is a gap in how routine testing works, and it leaves most sexually active adults carrying a virus they do not know about.

HPV is not rare or exotic. It is everywhere, including on Koh Samui, and it behaves very differently from the infections most tourists think to test for. No sores, no discharge, no obvious sign. For many people, especially men, there is nothing to see at all. What HPV actually is, what it can do, and what you can do about it right now on the island — that is what this article covers.

What HPV Actually Is and Why Most People Have Never Been Tested

Human papillomavirus is a family of more than 200 related viruses. Most are harmless. But a small group of high-risk strains can, over time, cause serious problems if the infection does not clear on its own.

It spreads through skin-to-skin sexual contact, not just penetrative sex. Condoms reduce risk but do not eliminate it. Someone can carry and transmit HPV without any visible symptoms, which is exactly why it moves through populations so quietly.

Most HPV infections clear on their own within one to two years. Routine testing of every sexually active adult would produce a large number of positive results for infections that would never cause harm. So standard protocols focus on specific groups where testing actually changes the clinical decision.

HPV in Thailand — Why the Local Strain Picture Matters

Not all HPV strains carry the same risk, and the strains circulating most widely in Thailand are not the ones Western health campaigns typically focus on.

Research conducted in Thailand and across Southeast Asia points to HPV types 16, 52, and 58 as the dominant high-risk strains in this region. Type 16 is well known globally. Types 52 and 58 are more common in Asia than in Europe or North America, and they are linked to the same serious outcomes: cell changes that can progress toward cervical cancer if left undetected.

This matters because older HPV vaccines do not cover strains 52 and 58. Gardasil 9, the newer nine-strain vaccine, does. If you are in Thailand and considering vaccination, or if you received an older vaccine series years ago, this is worth raising at a consultation.

High-Risk HPV — What Happens If It Does Not Clear

Most HPV infections, even high-risk ones, resolve without any treatment. Your immune system handles them quietly, and you never know they were there.

The problem is persistence. When a high-risk strain stays in the body for years, it can cause abnormal cell changes in the cervix. These changes are graded by severity. Mild changes often reverse on their own. More significant changes may need treatment before they progress further.

The critical point is that these changes are detectable. A Pap smear picks up abnormal cells. An HPV DNA co-test checks whether a high-risk strain is present. When caught early, outcomes are almost always good. The risk is not knowing.

Beyond Cervical Cancer — HPV-Linked Cancers Tourists and Expats Often Miss

Most conversations about HPV focus on cervical cancer. That is important, but it is not the whole picture.

HPV is also linked to throat cancer, specifically oropharyngeal cancer, as well as anal, vulvar, vaginal, and penile cancers. Oropharyngeal cancer from HPV has been rising in many countries, and it affects both men and women. Anal cancer linked to HPV is particularly relevant for men who have sex with men, where research in Thailand has found very high HPV prevalence. These cancers do not get discussed as often, but the virus behind them is the same one most people have never been tested for.

HPV and Men — Why Testing Is Different and What to Do

This is the question we hear most often from men who come in asking for a full STI screen. They want an HPV test. The honest answer is that there is no approved routine HPV DNA test for men without symptoms.

That is not because HPV does not affect men. It absolutely does. High-risk strains are linked to penile, anal, and oropharyngeal cancers. Low-risk strains, particularly types 6 and 11, cause genital warts. For men with visible warts, a clinical examination can identify and treat them. For men with no symptoms, a thorough visual examination is what we can offer.

What we can also offer is vaccination. Gardasil 9 is approved for men up to age 45, and getting vaccinated reduces the risk of both transmitting HPV and developing HPV-related disease. For any man who has not been vaccinated, this is the single most useful step available.

HPV Vaccination on Koh Samui — Who Should Get It and When

Gardasil 9 is available at Doctor Samui’s Chaweng clinic. The vaccine covers nine HPV strains, including the two high-risk strains prevalent across Asia, types 52 and 58, that older formulations miss.

The standard schedule is three doses given over six months. If you started a vaccine series at home and are partway through, you do not need to restart. Continuing from where you left off is clinically acceptable as long as the timing fits and the vaccine has been cold-chain stored correctly. We can provide WHO-compliant vaccination documentation if you need it for travel records or health insurance.

Who should get vaccinated? Anyone between 9 and 45 who has not completed a full Gardasil 9 series. People who are already sexually active can still benefit, since the vaccine covers strains you may not yet have encountered. Pregnancy is the main reason to delay. A quick consultation is the fastest way to confirm whether vaccination is right for your situation.

How HPV Testing Works at Our Clinic on Samui

For women, the standard approach is a Pap smear combined with an HPV DNA co-test. The Pap smear checks for abnormal cervical cells. The HPV DNA test checks directly for high-risk strains using a sensitive PCR method. Together, they give the clearest picture of your cervical health.

For men with visible genital warts, a clinical examination is the starting point. Treatment options depend on size and location, and those details are discussed at the time of your appointment.

Results from HPV DNA testing typically come back within a few days. We go through the result with you directly and explain what, if anything, comes next.

Frequently Asked Questions

Can I get an HPV test on Koh Samui as a tourist?

Yes. For women, a Pap smear with HPV DNA co-testing is available at the clinic. For men without symptoms, there is no approved routine HPV DNA test, but a clinical visual examination can be carried out and a vaccination consultation arranged. Same-day walk-in appointments are available in Chaweng, and hotel or villa visits can be arranged if you cannot travel easily.

Is the HPV vaccine available in Koh Samui, and can I continue my Gardasil course here?

Gardasil 9 is available at Doctor Samui’s Chaweng clinic. If you have already received one or two doses abroad, you do not need to restart the series. Call ahead to confirm stock, and we can provide WHO-compliant documentation for your vaccination records.

Why is HPV called the STI you have probably never been tested for?

Because it is not included in standard STI panels. Most infections cause no symptoms and clear on their own, so routine testing of everyone would generate large numbers of results that do not change clinical care. Most people carry HPV at some point without ever knowing, which is why this gap in testing awareness matters.

Which HPV strains are most common in Thailand, and does it affect which vaccine I should get?

Research in Thailand points to types 16, 52, and 58 as the dominant high-risk strains. Types 52 and 58 are more prevalent across Asia than in Western countries, and they are only covered by Gardasil 9, not by older vaccine formulations. This makes Gardasil 9 the preferred choice for anyone getting vaccinated in Thailand.

Can men get HPV on Koh Samui and what should they do about it?

Men can absolutely contract and transmit HPV. Most cases in men are asymptomatic, but high-risk strains are linked to penile, anal, and oropharyngeal cancers, and low-risk strains cause genital warts. Gardasil 9 is approved for men up to age 45, and a discreet consultation and vaccination can be arranged at the clinic or at your hotel.

I had unprotected sex on a trip to Koh Samui. Should I get an HPV test alongside my other STI checks?

An HPV DNA test for women can be added to a broader sexual health screen, and it is often a good idea alongside tests for chlamydia, gonorrhoea, HIV, syphilis, and hepatitis B. For men, a full sexual health consultation is the right starting point. Doctor Samui’s sexual health service covers all of this in one visit, at the clinic or at your accommodation.
