In the first 24 hours, some redness, mild swelling, and soreness right at the wound is completely normal. Your body is responding the way it should. The wound may throb a little, especially if it is on a hand or foot.

By 48 hours, that initial soreness should be starting to ease, not getting worse. Swelling should be stable or reducing. If things are moving in the wrong direction at 48 hours, that is your signal to act.

By 72 hours, a healing wound should look clearly better. Less red, less tender, with no discharge except a small amount of clear fluid at the surface. If it still looks angry at 72 hours, get it assessed. Do not wait for day four.

Dog Bite Infection Signs That Mean the Wound Is Getting Worse

Redness spreading past the wound edge is the first clear sign of infection. It creeps outward, sometimes in an uneven shape. If you drew a pen line around the red area this morning and it has gone past that line by evening, come in today.

Increasing pain after 48 hours is a reliable warning. A bite that was a four out of ten yesterday and is a seven today is telling you something. Pus or cloudy discharge, and skin that feels warmer than the surrounding area, both point to active infection.

Fever and chills mean bacteria have started moving beyond the wound itself. At that point, oral antibiotics alone may not be enough. Swollen lymph nodes in the armpit after a hand bite, or in the groin after a foot or leg bite, tell the same story.

The bacteria most commonly behind dog bite infections include Pasteurella, which causes rapid swelling and pain within hours, as well as Staphylococcus and Streptococcus species. A rarer but serious one called Capnocytophaga can affect people who are immunocompromised or who have had their spleen removed.

Soi dogs that live outdoors and scavenge carry a broader and less predictable mix of bacteria than a vaccinated house pet. That is not a reason to panic, but it is a reason to take even a small puncture seriously from the start.

The Rabies Risk: Why a Dog Bite in Thailand Is Not the Same as One Back Home

This is the part most tourists underestimate. Thailand has circulating rabies in its stray dog population. A bite from a dog with unknown vaccination status means post-exposure prophylaxis (PEP), a course of rabies vaccines, needs to start as soon as possible and ideally within 24 hours.

Rabies is almost always fatal once symptoms appear. PEP prevents it from reaching that point, but only if it is started in time. For deeper wounds or bites to the face, head, or hands, rabies immunoglobulin (RIG) is given alongside the first vaccine dose.

Where the bite lands matters. Hands have dense tendons, joints, and nerves close to the surface, and infection there can reach deep structures quickly. Bites to the face carry a higher risk of nerve involvement and a faster route that concerns doctors most, which is why PEP with RIG is prioritised for facial bites.

Feet and lower legs are where many Samui bites happen, from dogs approaching barefoot walkers on the beach or temple grounds. Poor circulation in the lower limbs and constant exposure to heat and ground-level bacteria make these bites higher risk for infection.

A bite on the upper arm or thigh is generally lower risk for structural damage, but infection risk is the same. Location affects urgency, but every bite needs proper cleaning and rabies assessment on the same day.
