Zepbound as a brand name is not separately registered in Thailand. The identical active drug, tirzepatide, is available here under the brand name Mounjaro, which launched in Thailand in 2025 with approved indications covering both type 2 diabetes and obesity management. If you are on Koh Samui and looking for tirzepatide, you can access it through a licensed prescriber at Doctor Samui.
You have been managing your weight with tirzepatide back home, your prescription ran out mid-trip, and now you are on the island with three weeks left and no plan to fly back early. Or you are a long-stay expat in Lamai who has been reading about Zepbound and wondering why every clinic you call draws a blank. The answer is not that tirzepatide is unavailable in Thailand. It is that it goes by a different name here.
Tirzepatide is sold in Thailand as Mounjaro, not Zepbound. Both contain the same drug at the same doses, and the Thai-approved label covers both type 2 diabetes and obesity. Whether you are starting fresh or continuing treatment from home, the access pathway on Koh Samui is straightforward through a licensed clinic.

Zepbound vs Mounjaro: Same Drug, Two Brand Names
Eli Lilly makes both. Mounjaro was approved first, for type 2 diabetes. Zepbound followed, approved by the US FDA in November 2023 specifically for chronic weight management. The active ingredient is tirzepatide in both cases, at identical doses and in the same injection pen format.
In Thailand, Eli Lilly and its regional distributor Zuellig Pharma registered and launched Mounjaro. Zepbound as a separate brand has no confirmed Thai FDA registration as of the date of this review. When you search "Zepbound Thailand" and find nothing at pharmacies or clinics, that is the reason. The drug is here. The brand name is not.
Thailand’s Regulatory Position on Tirzepatide
The Thai FDA approved Mounjaro for both type 2 diabetes and obesity management. Zuellig Pharma launched it in Thailand in 2025. It is a prescription-only medication. You cannot buy it over the counter or from an online seller and be on the right side of Thai law or your own safety.
Licensed private clinics can prescribe it after a proper consultation. A doctor needs to assess your BMI, review any existing conditions, and confirm there are no reasons to avoid it before writing a prescription. This is not a formality. There are genuine contraindications, and starting without follow-up creates real risks.

Who Qualifies for Tirzepatide in Thailand
The Thai-approved label sets out the eligibility criteria clearly. Adults with a BMI of 30 or above qualify. Adults with a BMI between 27 and under 30 also qualify if they have at least one weight-related condition: hypertension, abnormal cholesterol or triglycerides, obstructive sleep apnea, or cardiovascular disease.
Not everyone who meets those BMI numbers is automatically a suitable candidate. Tirzepatide is not appropriate if you have a personal or family history of medullary thyroid cancer, or a condition called multiple endocrine neoplasia syndrome type 2. Anyone with a history of pancreatitis needs careful review before starting. A face-to-face consultation is the right first step, not an online form.
How Tirzepatide Works for Weight Loss
Tirzepatide activates two hormone receptors at once: GLP-1 and GIP. Both play a role in regulating blood sugar and appetite. Targeting both reduces hunger, slows how fast the stomach empties, and improves how the body handles insulin. The result is a sustained drop in calorie intake alongside meaningful improvements in blood sugar control.
Clinical trial data from the SURMOUNT programme showed significant average weight loss in adults with obesity across all doses studied. The exact amount depends on the dose reached and how long treatment continues. Your prescriber can walk you through what is realistic for your starting weight and situation.
Dosing, Titration, and What to Expect on Samui
Tirzepatide is a weekly injection. Treatment starts at the lowest dose, 2.5 mg, to allow your body to adjust. The dose is increased at intervals set by your prescriber, typically every four weeks, until you reach an effective maintenance level. The maximum approved dose in the US and other markets is 15 mg weekly. In Thailand, Mounjaro pens are currently available up to 10 mg; your prescriber can advise on what strengths are accessible for your treatment plan.
The most common side effects are nausea, vomiting, diarrhoea, and reduced appetite. These tend to be strongest in the first weeks after a dose increase and settle as the body adjusts. Serious but rare side effects include pancreatitis and gallbladder problems. Severe abdominal pain that does not resolve warrants a same-day doctor review.
If you are continuing tirzepatide from a prescription started at home and your supply has run out on the island, do not skip doses without speaking to a doctor first. Stopping and restarting requires a medical review, and you may need to step back down to a lower dose before continuing.
Counterfeit and Unregulated Products: Why Source Matters Here
Online listings selling tirzepatide without a prescription exist in Thailand, including in tourist areas on Koh Samui. Compounded versions advertised at a fraction of the branded cost have no regulatory oversight for purity, dose accuracy, or sterile manufacturing. You could be injecting the wrong concentration, a contaminated preparation, or something that is not tirzepatide at all.
The health risk is real. Incorrect dosing can cause severe nausea and, for people on other diabetes medications, dangerous blood sugar drops. There is no medical follow-up built into an online purchase. Only obtain prescription GLP-1 medication from a licensed clinic where a qualified prescriber reviews your health history first. You can read more about accessing tirzepatide safely through the tirzepatide availability in Thailand page at Doctor Samui.
Getting Tirzepatide on Koh Samui
For long-stay expats managing their weight on the island, or travellers who started tirzepatide before arriving and need continuity of care, Doctor Samui’s Chaweng clinic handles tirzepatide consultations directly. The clinic also offers 24/7 hotel and villa visits across Koh Samui, from Lamai and Bophut to Choeng Mon and Maenam, so you do not need to make your way to Chaweng if you are staying elsewhere on the island.
A consultation covers eligibility review, dose assessment, prescription where appropriate, and follow-up planning. If you are already on a stable dose and need a prescription to continue your stay, bring documentation of your current regimen and the name of the prescriber who started your treatment. That makes the review faster and the prescription more straightforward.
Before starting any new prescription medication, a health check-up in Koh Samui at the clinic can provide a useful baseline, including blood pressure, blood sugar, and cholesterol. The contact page has direct options to reach the team by phone or message before you book.
Doctor Samui offers tirzepatide consultations at the Chaweng clinic and through 24/7 hotel and villa visits across Koh Samui. Whether you are starting treatment or continuing a prescription from home, contact Doctor Samui to discuss eligibility, dosing, and your next steps.
