Unopened Ozempic pens must be kept between 2°C and 8°C in the fridge. After your first injection, you can store the pen at room temperature between 15°C and 30°C for up to 56 days from that first use, then discard it regardless of how much is left. In Koh Samui’s heat, where outdoor air and parked vehicles regularly exceed 30°C, that upper limit is not a distant edge case. It is a daily storage challenge that needs a practical solution before it becomes a problem.
Your Ozempic pen made it through the airport, survived the ferry, and is now sitting in your villa at Chaweng while the afternoon heat climbs past 32°C. That is the moment most travellers realise the storage instructions on the box were not written with a Thai island in mind. The rules are simple on paper. The challenge is following them when the air conditioning goes off overnight, the minibar fridge runs colder than expected, or the pen spends twenty minutes in a bag on the way back from a beach lunch.
This guide covers the official temperature rules from Novo Nordisk’s prescribing information, what happens when those limits are crossed, and how to manage your pen safely while you are on the island. If you are already on a GLP-1 medication and want ongoing support, Doctor Samui’s GLP-1 service covers medical consultations for patients managing weight or blood sugar with semaglutide.

Before First Use: Unopened Pen Storage Rules
An unopened Ozempic pen belongs in the fridge. The required range is 2°C to 8°C, and that range matters at both ends. Too cold and the pen freezes. Too warm and the medication starts to degrade.
The fridge door is not the right place for it. Doors experience more temperature swings every time you open and close them. Store the pen in the main body of the fridge, away from the back wall and the cooling element. Direct contact with the cooling element is how pens freeze even inside a fridge set to a safe temperature.
Freezing causes permanent damage. Novo Nordisk states that a frozen pen should be discarded even after it has thawed. The liquid may look completely normal, but the semaglutide has been altered in a way you cannot see. Do not inject it.
After First Use: The 56-Day Rule for Ozempic Pen Storage
Once you give yourself the first injection, the clock starts. You have 56 days from that date to use or discard the pen. Not from when you opened the box. Not from when you first noticed the seal was broken. From your first injection.
If you inject weekly, 56 days covers eight doses on the 0.25 mg and 0.5 mg pens, which contain eight doses. The 1 mg and 2 mg pens contain four doses each, so those pens may be empty well before 56 days. Any pen in use past that point should be discarded, even if doses remain. Write the date of first use on the pen label with a marker. It is easy to forget when you are travelling.
After first use, the pen can be stored either at room temperature between 15°C and 30°C or back in the refrigerator between 2°C and 8°C. Keep the cap on and keep it away from direct sunlight and heat sources. In Koh Samui’s climate, returning the pen to a fridge when you are not travelling with it is a practical option the label allows.
| Storage Stage | Temperature Range | Time Limit | Notes |
|---|---|---|---|
| Before first use (unopened) | 2°C to 8°C | Until expiry date on box | Fridge interior only, not the door or near the cooling element |
| After first use (in use) | 15°C to 30°C or 2°C to 8°C (fridge) | 56 days from first injection | Keep cap on, away from direct sunlight; either location is permitted |
| Frozen at any stage | Discard | N/A | Do not use after thawing |
| Exposed above 30°C | Discard | N/A | Potency may be reduced even if the liquid looks normal |

What Happens When Your Ozempic Pen Gets Too Warm
Semaglutide is a peptide, a chain of amino acids. Heat causes peptide chains to lose their shape. Once that happens, the medication no longer works as it should. The liquid will look identical. There is no colour change, no cloudiness, no smell. You cannot tell by looking.
If your pen has been above 30°C, Novo Nordisk’s guidance is to discard it. Do not inject a pen left in a parked car, a sun-facing bag, or a room with no air conditioning. The financial loss is real, but injecting a degraded pen means taking your weekly dose with an unknown reduction in potency. That is not worth the risk if you are using Ozempic for weight management or blood sugar control.
If you are unsure whether your pen was affected, do not guess. Get advice from a doctor or pharmacist before using it.
Storing Ozempic in Koh Samui’s Heat and Humidity
Koh Samui sits in a tropical climate. Average outdoor temperatures run between 25°C and 35°C year-round. A parked car on the island can reach 50°C inside within minutes. A beach bag left in the sun is no different. The 30°C limit in the Ozempic storage instructions is crossed routinely here without people noticing.
A hotel minibar fridge is a workable option for your unopened pen, but check the temperature first. Some mini-fridges run colder than expected and will freeze the pen if it sits against the back wall. A small travel thermometer is one of the most useful items you can carry with an injectable medication in a tropical country.
For your in-use pen, the goal is to keep it in an air-conditioned space as much as possible. When you leave your room, use an insulated travel case with a gel ice pack. These are available from pharmacies in Chaweng and easy to order before you travel. They are not expensive. For patients working with Doctor Samui on storing Ozempic in hot climates, this is the single practical step that prevents the most waste.
Travel Storage Kit: What Actually Works
An insulated medication case with a reusable gel ice pack is the practical standard for travelling with Ozempic. A few things to know before you pack one.
Never let the ice pack touch the pen directly. A frozen or near-frozen surface can freeze the pen even during transit. Wrap the pen in a cloth or use a case with a built-in separator. If you are using a soft cooler bag, the same rule applies.
When you are out on a day trip to Angthong or a longtail trip up to the northern beaches, leave the pen at the hotel if you do not need it that day. Reducing the pen’s time outside a cool space is the simplest storage strategy available.
If you are staying somewhere without reliable air conditioning, ask the front desk about fridge access. Even a shared fridge at reception is better than leaving the pen in a warm room overnight.
When to See a Doctor About Your Ozempic Pen on Koh Samui
If your pen has been exposed to extreme heat, frozen, or you have lost track of when first use was, do not continue using it without advice. These are not bureaucratic rules. The consequence is a dose that may not work, which matters whether you are managing weight or blood sugar.
Doctor Samui offers medical consultations for patients on GLP-1 medications including Ozempic. Ozempic is not suitable for everyone. People with a personal or family history of medullary thyroid carcinoma, multiple endocrine neoplasia syndrome type 2, or a prior serious allergic reaction to semaglutide should not use it. Seek medical advice promptly if you develop severe abdominal pain, signs of an allergic reaction, or sudden vision changes while using this medication. If your pen has been compromised while travelling, a doctor can assess the situation, advise on whether to replace it, and help you stay on schedule. The clinic is based in Chaweng and covers the full island including Lamai, Bophut, Maenam, and Choeng Mon. If you need a doctor to come to you, 24-hour hotel and villa visits are available across the island. You do not need to interrupt your trip or wait until you get home.
For patients on longer stays, the health check-up service at Doctor Samui can also support ongoing medication reviews during your time on the island.
If your Ozempic pen has been exposed to heat, frozen, or you are unsure whether it is still safe to use, Doctor Samui can help. We offer medical consultations for patients on GLP-1 medications including semaglutide, with walk-in appointments at our Chaweng clinic and 24/7 hotel and villa visits across Koh Samui.
