Get off the foot. Sit on the beach or have someone help you to a dry surface straight away. Do not keep walking on it.

Rinse the area with clean water or seawater and look at what you are dealing with. Spines sitting above the skin surface that have not broken can be lifted out gently with tweezers or clean fingernails. The key word is lift. If a spine does not come away with gentle traction, leave it. Forcing it breaks it deeper into the tissue.

Soak the foot in hot water, as hot as is comfortable without burning, for 30 to 60 minutes. This eases pain and may help loosen very superficial spines. Beach restaurants and vendors usually have hot water on request. If there is obvious bleeding, light pressure with a clean cloth is fine. Do not pack or tape over puncture wounds.

Myths and Home Remedies That Make a Sea Urchin Sting Worse

Beach wisdom gets this injury badly wrong, and the consequences here are real.

Urinating on the wound: no benefit, skip it entirely.

Vinegar causes the most confusion. It is the correct first aid for jellyfish stings, where it stops venom-firing cells from activating. For sea urchin spines, the evidence that vinegar dissolves calcium carbonate is weak and applies only to the most superficial cases. It will not pull a deep spine out.

Lime juice is the one to remember specifically. It is everywhere on Koh Samui, and some beach guides recommend applying it to spine wounds. Do not. Spines in deeper tissue do not dissolve reliably. They migrate. They can travel toward tendons, joints, and nerves. Pain improving over the first 24 to 48 hours is normal. Pain that stays the same or gets worse after that is not.
