Wash First: The Single Most Important Step After an Animal Bite at Lamai Beach
Before you call anyone, get to a tap. Wash the wound with soap and running water for at least 15 minutes. Not a quick rinse under the beach-bar hose. Fifteen minutes, with soap, letting water run directly into the wound. This step alone cuts infection risk sharply.
Do not pour iodine straight into a deep puncture. Do not try to squeeze or "bleed out" the wound. Do not apply toothpaste, coconut oil, or any home remedy. These either do nothing useful or make clinical wound care harder later.
Cover the wound loosely with a clean cloth and get to a clinic. We go deeper at the clinic with irrigation and debridement. We also check whether the wound needs suturing, which a dog-bite wound often should not.
Animal Bite Risk Zones Near Lamai Beach
Most people who come to us after a Lamai bite can name exactly where it happened.
The beach road between Lamai market and the main beach strip has the highest soi dog density on this side of the island. Dogs rest under parked bikes and in the shade of food stalls. Children running past a resting dog, or cyclists moving quickly, are the most common trigger.
Hin Ta Hin Yai, the rock viewpoint just south of Lamai, draws macaque monkeys that have largely lost their fear of people. Do You Actually Need the Rabies Vaccine? WHO Exposure Categories Explained
Not every scratch is the same. The WHO divides animal exposures into three categories, and the category determines what treatment you need.
Category I means you touched an animal or were licked on unbroken skin. No vaccine is needed, but wash well. Category II means nibbling of uncovered skin, minor scratches, or small abrasions without bleeding. You need the vaccine course but not the immunoglobulin injection. Category III means one or more deep puncture wounds, bites that bled, scratches that broke skin and drew blood, or any contact with a bat. Category III requires the full vaccine course plus rabies immunoglobulin, called RIG, injected around the wound site.
The ten-day observation rule applies only if the biting animal is a domestic dog or cat that can be caught and confined. A soi dog on Lamai Beach Road cannot be observed for ten days. When the animal cannot be monitored, we treat as if it is positive and start post-exposure prophylaxis without waiting.
Monkey Bites Near Lamai Are Treated More Urgently Than Dog Bites
A scratch or bite at Hin Ta Hin Yai or Wat Khunaram raises the clinical priority above a dog bite of the same category.
Macaque monkeys can carry Herpes B virus, which is separate from rabies and rare but potentially serious in humans. It changes the assessment enough that we evaluate primate exposures faster. We do not alarm patients unnecessarily, but we do start that process immediately.
Parents especially need to know this: a scratch from a monkey counts the same as a bite. Saliva on the claws can carry virus. What the Rabies Vaccine Course Looks Like at Doctor Samui
The standard WHO post-exposure schedule for someone with no prior rabies vaccination runs on day 0, day 3, day 7, and day 14. Day 0 is the day of your first dose, regardless of when the bite happened. If you come in three days after the bite, your first appointment is still day 0.

Bitten Yesterday or Days Ago: Is It Too Late?
This is one of the most common calls we receive. Someone had a small bite or scratch two or three days ago, thought little of it, and is now awake at 2am after reading something alarming online.
It is not too late. Rabies has a long incubation period, often weeks or months, depending on where on the body the bite occurred. Post-exposure treatment started days after an exposure is still highly effective, provided symptoms have not begun. Once symptoms appear, treatment cannot reverse the disease. But that window between bite and symptoms is long enough that acting right now still makes a real difference.
