You noticed it this morning. A discharge, a burn when you urinate, or something that just does not feel right. You have a few days left on Samui, your usual doctor is back home, and you are quietly hoping it will go away on its own. It will not. Gonorrhea does not clear without treatment, and the good news is that treatment here is fast, private, and straightforward.

One injection. That is the current standard. A single intramuscular dose of ceftriaxone clears an uncomplicated gonorrhea infection. Most people feel significantly better within 24 to 48 hours. This article tells you what to expect before, during, and after.

What Gonorrhea Is and Why It Needs Proper Treatment

Gonorrhea spreads through unprotected vaginal, oral, or anal sex. Unlike a viral infection, it can be cured, but only with the right antibiotic at the right dose.

That distinction matters on Samui. Pharmacies here sell antibiotics over the counter, and some people pick up a random course hoping it will do the job. It usually does not. Gonorrhea has developed resistance to many older antibiotics, and using the wrong one does not cure you. It can reduce symptoms while the infection keeps progressing.

Left untreated, gonorrhea spreads. In men it can reach the testicle and surrounding tissue, causing significant pain and swelling. In women it can spread into the uterus and fallopian tubes, causing pelvic inflammatory disease, which can affect fertility. These are not rare outcomes. They are what happens when treatment is delayed or incorrect.

Symptoms — What Gonorrhea Looks and Feels Like

The most common symptoms in men are a yellow or green discharge from the penis and a burning sensation when urinating. These usually appear two to seven days after exposure and are hard to ignore.

In women, symptoms are often milder or absent entirely. A large number of women with gonorrhea have no symptoms at all, or notice only mild discharge and pelvic discomfort they put down to something else. This is one of the main reasons the infection spreads so easily.

Gonorrhea also infects the throat and rectum. A throat infection from oral sex rarely causes noticeable symptoms. Rectal gonorrhea can cause discharge or discomfort, but is often silent too. If your exposure involved oral or anal sex, a throat or rectal swab may be needed alongside a urine test.

Testing First — Why Confirmation Matters Before Treatment

Burning when you urinate and a discharge could be gonorrhea. It could also be chlamydia, or both at once. It could be non-gonococcal urethritis, which does not respond to ceftriaxone at all. Treating the wrong infection with the right drug still leaves you unwell.

We use a PCR urine test that picks up gonorrhea and chlamydia from the same sample. Results take 24 to 48 hours. If your symptoms are clear and your travel window is tight, treatment can be given the same day and confirmed when the result comes back.

A PCR test is reliable from around one to two weeks after exposure. If you were exposed more recently than that, we test and retest once the window has passed. The Treatment — One Ceftriaxone Injection

The current first-line treatment for gonorrhea is a single 500mg intramuscular injection of ceftriaxone. This is the protocol recommended by the CDC and WHO. The injection goes into the upper outer buttock or thigh. It takes about 30 seconds.

Dual therapy with azithromycin alongside ceftriaxone used to be standard. It is no longer recommended. Azithromycin resistance has risen sharply, and adding it now offers no benefit. Ceftriaxone alone is what works.

If you have a cephalosporin allergy, mention it before the injection. Ceftriaxone belongs to that antibiotic family, and alternative regimens exist, though they require more doses. This is why the consultation happens before the injection.

If your chlamydia result is pending or comes back positive, a seven-day course of doxycycline is added to cover that separately. One injection covers one infection.

Why One Injection — Resistance in Southeast Asia

Resistance to older drugs like penicillin, tetracycline, and fluoroquinolones is well documented across the region. Ciprofloxacin, cheap and easy to find over the counter in Thai pharmacies, does not reliably treat gonorrhea here anymore.

This is not abstract. We see patients at the Chaweng clinic who have already taken pharmacy antibiotics, feel partially better, and then discover on testing that they still have an active infection. They have taken the wrong drug, the infection has had more time to cause damage, and they have potentially passed it on.

After Your Injection — What to Expect

Most patients feel noticeably better within 24 to 48 hours. Discharge reduces. Burning improves. By day three to five, symptoms are usually gone.

Avoid sex for at least seven days after your injection, and only once all symptoms have completely cleared. If your partner has not been treated, that seven-day window resets every time you have unprotected contact with them. You can reinfect yourself from an untreated partner within hours of your own injection.

If symptoms are not improving after five days, come back in. A follow-up PCR test to confirm the infection has cleared is recommended if symptoms persist or if a drug-resistant strain is suspected.

Your Partner Needs Treatment Too

If you have gonorrhea, your partner almost certainly has it, whether or not they have symptoms. Partner notification is not optional.

If your partner has already flown home, they should see a doctor as soon as they land and mention the potential exposure. They do not need to wait for your confirmed result.

Risks of Leaving Gonorrhea Untreated

Untreated gonorrhea does not stay in one place. In men it travels upward to the testicle and surrounding tissue, causing significant scrotal pain and, in some cases, lasting damage to sperm production. In women it spreads into the uterus and fallopian tubes, causing pelvic inflammatory disease, which can lead to infertility or ectopic pregnancy.

In rare cases the infection enters the bloodstream, causing joint pain, skin lesions, and in serious cases, infection of the heart valves. That requires hospital treatment.

None of this is scaremongering. It is what happens when a curable infection goes untreated for weeks. The injection takes 30 seconds. The complications take much longer to deal with.

Almost certainly not. Gonorrhea in Southeast Asia has become resistant to many commonly available antibiotics, including ciprofloxacin, which is easy to find over the counter at Thai pharmacies. Taking the wrong drug can reduce symptoms temporarily while the infection continues progressing.

My partner is still here on the island — do they need to be treated too?
