What Chlamydia Does and Why So Many People Miss It
Chlamydia is quiet. A large proportion of people who carry it have no symptoms at all. In women, there is often no discharge, no pain, nothing obvious. In men, burning urination or discharge may appear, but plenty of men feel nothing either.
This is exactly why it spreads so easily on holiday. You feel fine. Your partner felt fine. Neither of you had any reason to test. If you have been sexually active on Koh Samui without consistent condom use, asymptomatic chlamydia is worth ruling out.

Which Antibiotic You Will Be Given for Chlamydia Treatment in Koh Samui
The short answer: most likely doxycycline. Since updated CDC guidance in 2021, doxycycline 100mg twice daily for seven days is the preferred first-line treatment. Azithromycin 1g as a single dose is still effective for straightforward genital chlamydia, and we use it in certain situations. For rectal chlamydia, doxycycline is clearly more effective, and the evidence now favours it as the stronger option overall.
If you are pregnant, the decision is different. Azithromycin is currently considered the safer choice in pregnancy, but this depends on a proper clinical assessment. If there is any chance of pregnancy, say so at the start of the consultation.

How Soon Does Chlamydia Treatment Work — the Honest Timeline
This is the question we hear most. Here is what actually happens.
Within 24 to 48 hours of your first dose, most patients notice symptoms beginning to ease. Burning improves. Discharge reduces. That improvement is real, but it does not mean the infection is gone.
By day seven, if you have completed the full doxycycline course without missing doses, the infection should be eradicated. Azithromycin works over roughly the same window even though you only take it once. The drug stays active in your tissue for several days after that single dose.
Do not have sex, including oral sex, for seven days after finishing treatment. If your partner has not been tested and treated, you will reinfect each other. Reinfection is the main reason people end up back at the clinic.
After Treatment — Rules That Actually Matter on a Beach Holiday
Finish the full course. People feel better after two days and stop taking the tablets. Do not do that. The bacteria need the full seven days to be cleared properly.
Doxycycline makes your skin significantly more sensitive to the sun. On a beach holiday in Thailand, this is not a minor side note. You can burn badly and quickly, even with sunscreen. Cover up more than usual, avoid peak sun hours, and do not skip SPF. We see patients every season who spend a day on Chaweng Beach on doxycycline and pay for it.
Heavy alcohol use while taking doxycycline reduces how well the drug works. A few drinks is likely fine, but drinking heavily across a seven-day course is a real risk to your treatment outcome. For azithromycin, alcohol is less of a concern for the drug itself, though it can worsen nausea in the first 24 hours.
Take doxycycline with food and a full glass of water. It can irritate your throat and stomach if you take it lying down or on an empty stomach. Do not take it right before bed.
Why Getting Treated Promptly on the Island Matters
Untreated chlamydia does not stay quiet. In women, it can move into the uterus and fallopian tubes, causing pelvic inflammatory disease. That means pelvic pain, fever, and in some cases longer-term effects on fertility. In men, it can cause epididymitis, which is painful inflammation of the tube behind the testicle.
Neither complication is inevitable, and both are avoidable if you treat early. Getting antibiotics on day one of your holiday is far better than landing home with a two-week-old untreated infection.
