Emergency treatment in Koh Samui ranges from around 500 THB for a basic wound dressing at a walk-in clinic to several hundred thousand baht for a hospital admission or medical evacuation. For the most common injuries tourists face here — road rash, coral cuts, jellyfish stings, dog bites, and dehydration — a walk-in clinic is clinically appropriate and costs a fraction of what a private hospital charges. Doctor Samui offers transparent, same-day pricing at our Chaweng clinic and 24/7 visits to hotels and villas across the island.
You came off your scooter on the ring road near Lamai, or you sliced your foot on coral during a snorkeling trip off Chaweng. Either way, you are sitting in your hotel room right now wondering what this is going to cost. That is exactly the question we hear every day, and the honest answer is: it depends a lot on where you go first.
Most tourists do not realise that going straight to a large private hospital for a non-life-threatening injury can cost three to five times more than the same treatment at a walk-in clinic. Koh Samui has no public healthcare system for visitors and no reciprocal agreements with foreign health services. Every baht comes out of your pocket or your insurer’s. Knowing your options before you go anywhere saves real money and a lot of waiting time.

Why Emergency Costs Feel So Different Here
Back home, you probably do not think about what a wound dressing costs. Here, there is no system catching you. Thailand does not have reciprocal healthcare arrangements with most countries, so tourists pay full private rates from the first consultation.
That does not mean care is poor or expensive by global standards. It means costs vary enormously depending on where you go. A walk-in clinic and a private hospital can treat the same coral cut for very different prices. The cut is the same. The bill is not.
The tropical climate also changes the stakes. A small wound on Koh Samui can develop an infection faster than you would expect in a cooler climate. What looks minor on day one can be red, swollen, and spreading by day three. Catching it early at a clinic is always cheaper than dealing with a serious infection later.
Common Emergency Treatment Costs in Koh Samui
These are the cases seen most often at our Chaweng clinic. The prices below reflect typical clinic-level costs, with private hospital costs shown for comparison.

|---|---|---|
| Road rash wound clean and dressing | 1,500 – 4,000 | 6,000 – 15,000+ |
| Coral cut treatment and dressing | 800 – 2,500 | 5,000 – 12,000 |

| Dog bite wound care and rabies PEP initiation | 2,500 – 6,000 | 8,000 – 20,000+ |
| IV drip for dehydration or food poisoning | 1,500 – 4,000 | 6,000 – 15,000 |
| Tetanus shot | 500 – 1,200 | 1,500 – 3,500 |
| Sutures for laceration | 2,000 – 5,000 | 7,000 – 18,000 |
| Follow-up wound dressing | 300 – 800 | 1,500 – 4,000 |
Road rash from scooter accidents — what people on the island call a "Samui Tattoo" — is the single most common injury treated here. It needs thorough cleaning to remove grit and road debris, or it will infect. At a clinic you pay for the care itself. At a private hospital ER you also pay a facility fee, an ER surcharge, a separate doctor fee, and a materials fee, all stacked on top.
Dog bites need particular attention. If there is any broken skin, you need post-exposure prophylaxis, known as PEP, for rabies — a series of injections started as soon as possible. Doctor Samui initiates this and gives you a schedule you can continue if you leave the island. Do not put it off.

Private Clinic vs Hospital: The Real Cost Difference
The biggest cost driver on Koh Samui is not the treatment itself. It is the facility you choose.
Bangkok Hospital Samui is well-equipped. For genuinely serious emergencies — a suspected fracture needing surgery, chest pain, loss of consciousness — it is the right place. For a coral cut, a jellyfish sting, a fever, or road rash, going there means paying ER fees and facility surcharges that can turn a 2,000 THB problem into a 12,000 THB bill before anyone has examined you.
A walk-in clinic handles the full range of common holiday emergencies without that overhead. The care is clinically equivalent for these injuries. The price is not. For accident and injury treatment, the clinic route is almost always the right first step.
How Travel Insurance Works Here
Most outpatient clinics in Thailand, including Doctor Samui, work on a pay-and-claim basis. You pay at the time of treatment and submit documents to your insurer for reimbursement afterward.
Before you leave the clinic, ask for two things: an itemised receipt showing every charge separately, and a medical certificate stating your diagnosis and what treatment you received. Most insurers will not process a claim without both documents. Doctor Samui provides them as standard.
Direct billing, where the insurer pays the clinic directly, is sometimes available at major private hospitals for inpatient stays on comprehensive policies. For outpatient visits it is rarely available anywhere on the island. Assume you will pay first and claim later, and keep every piece of paperwork.
What If You Have No Travel Insurance
More people arrive in Koh Samui without travel insurance than most clinics will admit. Long-stay expats, last-minute travellers, people who meant to buy a policy and forgot.
Out-of-pocket treatment at clinic level is affordable for most common injuries. A coral cut with dressing runs 800 to 2,500 THB. An IV drip for food poisoning is 1,500 to 4,000 THB. A dog bite assessment with the first rabies PEP injection is around 2,500 to 4,000 THB depending on the wound. These are real numbers.
The same treatment at a private hospital ER runs three to five times more. For uninsured visitors, the choice of where to go first makes a significant financial difference. Doctor Samui charges transparent rates with no hidden facility fees.
If an admission or surgery becomes necessary, costs escalate quickly. Medical evacuation by air ambulance can reach hundreds of thousands of baht. This is the scenario where not having insurance becomes genuinely serious.
When You Actually Need to Go to Hospital
A clinic handles most of what goes wrong on a Koh Samui holiday. But some situations need more, and we will always tell you when that is.
Go directly to hospital for a head injury with any loss of consciousness, chest pain or difficulty breathing, a suspected fracture that may need surgical fixation, a suspected spinal injury, a severe allergic reaction not responding to initial treatment, or any condition where you feel you are deteriorating fast. In these cases, do not start with a clinic.
For everything between "fine, just a scratch" and "call an ambulance," a walk-in clinic is usually the right first call. We treat, we stabilise, and we refer when the situation demands it.
