PEP — If You Think You Were Exposed in the Last 72 Hours

Stop here if your exposure was within the last 72 hours. PEP, short for post-exposure prophylaxis, is a short course of medication that can prevent HIV infection if started quickly enough. The 72-hour window is a hard limit. After that, it is no longer effective.

The sooner you start, the better it works. Do not wait until morning. Do not wait until you feel certain. If there is any realistic chance of exposure, come in now or call us for a hotel visit. Types of HIV Tests Available in Koh Samui

There are three main types of HIV test. Each one detects different things and becomes reliable at a different point after exposure.

Rapid Antibody Test

This is the finger-prick test that gives results in about 20 to 30 minutes. It detects antibodies only. Your body takes time to produce enough antibodies to show up on a test, so this option is not reliable until around 90 days after exposure. It works well for people who had a potential exposure more than three months ago and want a quick, clear answer.

4th Generation Antigen/Antibody Combo Test

This is the lab-based test we most commonly recommend. It detects both antibodies and the p24 antigen, which the virus produces very early in infection. Because it catches both, it becomes reliable much sooner, generally from around 18 to 45 days after exposure. A negative result at 45 days is considered highly reliable. This is the standard choice for most people who come in after a recent exposure.

HIV RNA PCR Test

This test looks for the virus’s genetic material directly. It can detect HIV from around 10 to 14 days after exposure, making it the earliest option available. It is more expensive and takes longer to process, but it is the most sensitive test we have. Some people use it as part of a same-day assessment alongside PEP when the exposure was very recent.

Understanding the HIV Window Period

The window period is the gap between exposure and when a test can reliably detect infection. Every HIV test has one. Testing inside that window does not prove you are negative. It proves the test could not find anything yet.

Your body can take weeks to produce detectable antibodies. Before that happens, an antibody-only test will read negative even if HIV is present. This is why timing matters, and why repeat testing is sometimes needed. A negative PCR at two weeks is a strong early signal, but we still recommend confirming with a 4th generation test at 45 days.
